Social Support Officer (Internal Applicants Only)

Join our Ageing Well team in the temporary position of Social Support Officer! In this role, you will be responsible for leading and enhancing the Social Support Group program, while also contributing to the initiatives and activities of the Social Connections Officer. The role will oversee, support and supervise volunteers and participants within the program, while working closely with the Social Connections Officer to strengthen social connections across the community.

What a typical day will look like;

  • Monitor and action referrals/ enquiries received through either the My Aged Care portal or from the community.
  • Onboard new clients, conduct service specific assessments, care planning and reviews.
  • Train, mentor and support the recruitment and retention of volunteers.
  • Co-design program activities with participants (within funding budget).
  • Adhere to and continuously improve to meet and comply with the New Aged Care Act
  • Work closely with the Social Connections Officer to run social activities as part of the Social Support Group and Social Connections program.

What you will bring to the role;

  • Cert IV in Community Services, Social Sciences or related field desirable but not essential
  • First Aid Level 2 certificate
  • Valid driver's licence
  • Experience and knowledge working with older people with a range of care needs
  • Demonstrated experience in planning and evaluating programs
  • Demonstrated ability to work co-operatively with volunteers, service users, colleagues and other organisations in order to achieve Council objectives
  • A sound understanding of the aged care reforms and Commonwealth Home Support Program
  • Demonstrated ability to be flexible and responsive to meet the needs of the services, volunteers and the community.
  • Ability to manage, set priorities, plan and organise own work to deliver within agreed timelines
  • Capable of supervising volunteers, including the ability to implement personnel policies and procedures and provide workplace training.
